Concealed Handgun Permit – Duplicate

Duplicate permits may only be ordered if your current permit is lost or destroyed. (NCGS14-415.11d)

The hours are as follows:

• Monday, Tuesday, Thursday, Friday
  8:30 a.m. – 3:30 p.m. (excluding Holidays)

Location

Application for a duplicate CHP takes place in person at the Forsyth County Public Safety Center located at 301 N. Church Street, Winston-Salem, NC 27101.

Applicant must provide a valid N.C. Driver's License or N.C. Identification Card at the time of the application.

Fee

Duplicate - $15.00

Method of Payment

Cash or Credit/Debit cards are accepted.
